Mail us your signature:

1. Download the petition form below.

2. Print the form as an EXACT COPY of the PDF. It must be double-sided. No photo-copying.

3. Legibly sign and print your name in the box under “I. SIGNATURE.”

4. Write your street address in the box under “II. NOW REGISTERED AT.”

5. Write the name of the city/town on the bottom of the page in the designated box.

6. Make sure the page has no stray marks, food stains, etc.

7. Ask your family, friends, and co-workers to do the same! Just maintain separate signature sheets for each city/town.

8. By June 8, mail your petition forms to 1105 Massachusetts Ave. #4B, Cambridge, MA, 02138. Or coordinate a pickup time by calling or texting 781-697-7919. This address and phone number also appear on the petition forms themselves.
